Lil’ Extreme Registration

In consideration of being allowed to participate in athletic programming, and all related sanctioned activities and events at the Lil’ Extreme at Saint Genevieve High School, on behalf of the following RELEASEES (Extreme Soccer Club), the undersigned acknowledges, appreciates and agrees that: Participation in the league inherently includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL SUCH INHERENT RISKS, both known and unknown, even if ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation. I willingly agree to comply with the stated and customary terms and conditions for participation in Lil’ Extreme League as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. I agree before I enter the Facilities that I have taken my temperature and it is less than 100.4 degrees Fahrenheit and that I have not tested positive or been in contact with anyone who has tested positive for COVID-19 within two weeks of entering the Facilities. While at the Facilities, I will wear a face covering when practical and will to the best of my ability practice social distancing. I, and my parent or legal guardian if I am a minor, for and on behalf of myself and my heirs, executors, administrators, successors, and assigns, hereby forever release, acquit, and discharge and AGREE TO INDEMNIFY AND HOLD HARMLESS all RELEASEES, their officers, directors, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the League from any and all causes of action or actions, suits, losses, liability, damages, claims (including but not limited to, negligence, gross negligence, willful or intentional conduct), personal injury damages, medical or hospital bills, property damages, judgements, levies, and executions which the undersigned or his/her heirs, executors, administrators, successors and assigns may have against said RELEASEES arising out of my participation in the League programming. I, and my parent or legal guardian if I am a minor, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I UNDERSTAND AND AGREE WITH EACH AND EVERY MATTER STATED HEREIN. I KNOWINGLY AND WILLINGLY CONSENT TO MY CHILD'S PARTICIPATION IN LIL’ EXTREME LEAGUE, AND I HEREBY BIND MYSELF AND MY MINOR CHILD TO THE TERMS OF THE WAIVER BY SIGNING BELOW.
